I have this weird habit of adjusting patient notes when I think something needs a tweak. Now, don't get me wrong, it's not anything major, like changing what they ate or their vital signs. No, it's more about making them sound better to the medical staff reviewing them. For instance, if someone is listed as having had a 'difficult night,' I might edit that to say they were 'restless but stable.' It feels like I’m improving the narrative of their care. I know this isn’t technically right, I’m supposed to stick to facts and observations only. But seeing those words make a difference in how we perceive someone’s condition, it's hard not to feel like I'm making things better somehow. This isn't about covering up mistakes or being deceptive; it's more of an effort to smooth over the rough edges of our documentation. It feels dishonest but also oddly comforting. I suppose the real issue is that I’m trying to fit into a role where perfection seems expected, and sometimes bending the rules feels like my way of contributing positively in a place that can be overwhelming.
